What is the purpose of the data collected on the clinical record?
The data collected at the Hugo Madeira Clinic is very important, not only for diagnosis but also for future comparison. Furthermore, it can also be used in article publications or presentations for scientific purposes — if the patient consents to this in writing.
Acting in accordance with the provisions of article 30 of the code of ethics of the Portuguese Dental Association, “the dental health care provider must have an archive containing all the individual clinical files of patients, regardless of their format, which must be kept for a minimum period of 20 years”, where “the individual clinical file comprises the clinical record and all the auxiliary means of diagnosis, including photographs, models, imaging exams and all sorts of patient information gathered during the diagnosis and treatment period”. In addition, “the dentist must develop and keep up to date an individual clinical file for the patient, containing, in detail, besides the identification of the dentist who carried out the treatment, personal data, health history, clinical observations, diagnosis, treatment plan and treatments carried out, expressed whenever possible using the Portuguese Dental Association Table of Nomenclature as a reference”.
The Hugo Madeira Clinic also follows all data protection under Portuguese law and is registered with the CNPD – National Commission for Data Protection, with authorisation nº 13825/2017 for process nº 6075/2014.
Can I make a teleconsultation?
Yes, you can have a teleconsultation with one of the clinicians on the medical team responsible for diagnostic and planning consultations.
During the teleconsultation, clinical questions can be clarified and, eventually, a provisional treatment plan can be drawn up, which will allow your Smile Manager to provide you with an estimate of the costs for your treatment.
Following the provisions of article 19 of the OMD code of ethics, “the telemedicine consultation does not replace the face-to-face relationship between the Dentist and the patient, and must be carried out under conditions that can be superimposed on a face-to-face consultation”. However, since the Dentist, when using telemedicine, does not observe the patient in person, he is always carrying out a preliminary and never definitive assessment.
Is it possible to get information on the price of the procedures by telephone?
Depende.
